Case Study

A parent arrives in the morning and gives you a sachet of Calpol. She completes your medication form and writes that the dose to be given is 1.25 mls. The practitioner who gives the medicine does not look at the sachet to see how much medicine is in a full sachet. The practitioner administers the full sachet then sees that the amount she has given is 2.5 mls, double the dose requested by the mother. The practitioner immediately phones the parent and tells her that she has overdosed her child.
