Support care plan activities

Case Study

Tahlia is a 56 year old lady who is recovering from a stroke. Until her stroke she was employed full time and her job involved driving. She is now at home and lives with her son who works full time. Ulrica, Tahlia’s carer, visits her at lunch times to ensure she has a meal. Tahlia has an alarm pendent around her neck if she needs assistance.During one of her visits Tahlia says she feels very frustrated and is going to start going out to the shops and doing things on her own. Ulrica explains to Tahlia that she needs to get others involved to ensure Tahlia’s safety.
  1. If you were Ulrica who would you contact to assist Tahlia to achieve her goals?
  2. Does Tahlia’s care plan need reviewing?
  3. Who needs to be involved in creating the new care plan and in providing support?
It is rare that there is just one way to respond to a scenario or situation, so do not consider that the response you have chosen to this scenario will be judged as right or wrong. Share your scenario with a work colleague and ask them to tell you if they think you have responded appropriately. Ask them to challenge your response if necessary. You do not need to submit this task for assessment unless asked to do so by your tutor.
