Working in partnership with others

Imran has just come out of hospital after a fall.

You are on duty when he arrives home and are responsible for communicating this and any changes to his care needs to his support team. On his discharge letter, it says his wound on his leg needs cleaning twice a day (morning and evening) and cream applying. He also has antibiotics to take for 2 weeks.
  1. Who do you would need to communicate with about this?
  2. How should I communicate this message, and what are the key points to communicate?

For example, you need to call Imran’s daughter and let her know that Imran is home from hospital, he has a wound on his leg that he has cream for and antibiotics for two weeks, and the care staff will help him with this.
It is rare that there is just one way to respond to a scenario or situation, so do not consider that the responses you have to the above scenario will be judged as right or wrong. Share your scenario with a work colleague or friend and ask them to tell you whether they think you have responded appropriately. Ask them to challenge your response if necessary. You do not need to submit this task for assessment unless asked to do so by your assessor.
